Your privacy is protected under the Privacy and Personal Information Protection Act 1998.
Personal information may only be disclosed in accordance with the exemptions provided under the Act.
We will only use the personal details required on this form for administration purposes related to the facility or service for which it is being completed and when we communicate information to you regarding any of Little Fins’s facilities, programs, services promotions that are deemed appropriate to your interests and needs and which may enhance your use of Little Fins Swim School. We will not make this information available to any third party unless it is necessary as part of the provision of this product or service. By signing this form you are providing us with consent to use the personal details for such purposes.

DIRECT DEBIT PAYMENT/ENROLMENT

During the Term Enrolment Rollover, fees are processed on the first day of each month. Payments are automatically deducted via Direct Debit. You will receive notification two days before the Direct Debit deduction is scheduled to take place.
It is the customer's responsibility to ensure that sufficient funds are available in their account to cover the payment of fees for the upcoming month. The customer may be liable for any dishonour fees charged if their financial institution returns a Direct Debit payment unpaid.

All payment details are managed by the customer through the Customer Portal. If your Direct Debit information changes, you must update your payment details in the Customer Portal before the next scheduled payment.

If a customer wishes to dispute a Direct Debit payment, they should contact their financial institution and/or Littlefins Swim School as soon as possible. We will make every reasonable effort to investigate and resolve the dispute within five (5) business
days. If the disputed payment is determined to be a valid debit, the customer may be liable for any chargeback or dispute fees incurred.

Littlefins Swim School does not offer refunds for fees paid under any circumstances.

MAKE-UP CLASS POLICY

To be eligible for a make-up class, the following conditions apply:

1. Advance Notice Required
Students must be marked absent through the online Customer Portal or app at least four (4) hours prior to the scheduled class start time.

2. Active Enrolment
The student must have an active ongoing enrolment, with all tuition fees paid and no outstanding account arrears.

3. Eligible Classes Only
Make-up classes are available for group Learn to Swim lessons only and do not apply to:
o Private lessons
o Holiday intensive programs

4. Customer Responsibility
It is the customer's responsibility to:
o Cancel their child's lesson through the online Customer Portal or app;
and
or Book an available make-up class using the online Customer Portal or app.

5. Make-Up Class Limit
A maximum of one (1) make-up class per month may be scheduled per student.

Please note: Make-up classes are subject to availability and are not guaranteed at preferred days or times.

PLAY BY THE RULES

By making payment and attending lessons, you agree to the following rules and conditions:

1. Arrive no more than 10 minutes prior to your lesson to avoid unnecessary congestion and waiting times.
2. Swim and go. Please leave promptly after your lesson to assist with facility operations and class turnover.
3. Long hair must be tied back to prevent hair from obstructing a student's vision during lessons. Swim caps are highly recommended.
4. Band-Aids and adhesive dressings must be removed before the lesson if there is a risk they may come off in the pool.
5. No food is to be consumed within the swim school to maintain a clean and hygienic environment.
6. No pets are permitted on the premises.
7. Respectful behaviour is expected at all times towards our staff, instructors, students, and other families.
8. Babies and toddlers under three (3) years of age must wear a Happy Nappy or reusable swim nappy over their disposable swim nappy during lessons to help maintain pool hygiene.

Aggressive, abusive, threatening, or disrespectful behaviour towards our staff will not be tolerated and may result in the suspension or cancellation of lessons and services.

By signing up for Direct Debit, your future invoices will be paid automatically on their due dates, helping you avoid late fees and missed payments.

Direct Debit payments are deducted on the first day of each month, and a reminder notification will be sent two (2) days prior to the scheduled deduction.
