Photography & Video Permission: By signing this form I hereby grant permission for my child to be photographed and/or videoed by Level-up Acrobatics. I understand these items may be used for promotional advertisement and be uploaded to LA’s Facebook page/websites/social media platforms.

Fees: By signing this form I understand that all LA’s fee requirements must be paid in full by the due date provided by LA. If this fails to occur you will receive a reminder of the outstanding balance. If the overdue fee continues, you understand there will be a 10% late fee added to all outstanding invoices and your child's classes may also be suspended until the amount has been paid in full. LA does not offer any refunds.

Student Behavior: By signing this form I understand as part of myself/my child's training the LA student code of conduct is to be withheld. This includes respecting other students and staff members, being honest and considerate, displaying good general behavior and treating everyone as equal.

Student Dance & Acro waiver: By signing this for I, the undersigned parent or legal guardian, do hereby grant permission for my son/daughter to participate in any classes/events with LA. I further acknowledge, understand and agree that by participating in the sport of Acro and Dance, there is a possibility of physical illness or injury (minimal, serious and catastrophic) and that my son/daughter is assuming the risk of such injury by participating. I authorize any representative of LA or the event party to consent and authorize any medical attention, treatment, surgery or administration of medication by qualified and licensed medical personnel for my son/daughter, which may become necessary. I understand I will be notified as soon as possible in the event of an emergency. I understand and agree that all expenses of such treatment are my responsibility. I agree to protect, defend, indemnify and will not hold LA, staff inclusive, from any and all claims, demands, losses, suits, liabilities, costs or other damages including court costs and attorney’s fees, arising from any injury to, or damage to or destruction of property arising out of or in connection with any damage to third parties occasioned by, incident to, arising out of, or in connection with my son/daughter's participation with LA.
