* I understand that all students must be registered through the PDPA CUSTOMER PORTAL before participating in any class
*I understand that enrolment from Term 1 will be carried across to further terms unless notified in writing by the parent that they wish to discontinue membership. To avoid liability for term fees notification must be prior to the start of term in most cases.
*I understand that should I wish to discontinue membership after the commencement of a term, I am liable for the full term payment.
*Payment for all monies requested by PDPA as part of the students enrolment is via WORLD PAY or Internet transfer. I may either pay in full by the due date through the World Pay link provided, or directly by internet transfer using BSB AND ACCOUNT NUMBER provided on my invoice or, if wishing to pay in installments, I must enter into a Direct Debit agreement with (WORLD PAY) Payments are spread across 3 installments - 1st - Friday prior to start of new term, 2nd and 3rd payments at 4 weekly intervals thereafter. My invoice will include installments for costume fund and admin fee covering licencing and insurance.
* COMPETITION ENTRY FEES or other non tuition fees will be invoiced separately.
*Customers must pay the WORLD PAY processing fees, these are shown on your WORLD PAY registration form and are dependent on whether you use a VISA/MASTERCARD or BANK ACCOUNT. Credit cards attract lower fees.
*If your direct debit fails the customer is responsible for paying the payment fail fee, which is charged by WORLD PAY. This amount is provided to you on the Direct Debit registration form. If your payment fails it will be automatically re-scheduled for 3 days after the original payment date.
PHOTOGRAPHY:
Photography and video may be taken during PDPA classes. Such media might be used for promotional purposes, including but not limited to reproduction on the PDPA social media platforms. By enrolling in PDPA classes you agree to such use by PDPA unless you notify us otherwise.
FIRST AID AND EMERGENCY PROCEDURES:
All PDPA STAFF are trained in first aid including use of a Defibrillator. In case of an emergency PIVOTAL DANCE PERFORMANCE ACADEMY will first contact the parent and then seek medical advice, the expenses of which will be paid by the parent. Due to the nature of dance training, some physical contact may be necessary by a teacher to demonstrate exercises or techniques during class or if there is a need to administer first aid.

CODE OF CONDUCT/ CHILD SAFE ENVIRONMENT POLICIES:
Our full policies are provided through our HANDBOOK provided to each family upon enrolment. For purposes of this brief statement:
PDPA maintains the right to un-enrol any student and remove any family from our centres or events hosted by PIVOTAL DANCE PERFORMANCE ACADEMY. This condition will be instated in the event that a student or family disregards our published Values or Code of Conduct including but not limited to a threat, intimidation, bullying or assaulting any staff member or other students or families. In the event this occurs no refunds will be issued for classes missed or any fees/invoices paid for at that time.
By submitting this enrolment, you agree you have read and understood our terms and conditions, give consent for your child or children to participate in a PIVOTAL DANCE PERFORMANCE ACADEMY class/es and understand the risk involved. You agree to accept full responsibility for any injury sustained to you or your child/children while attending classes, for any injury sustained by you, your child/children in the waiting areas, and for any loss or damage of property. You agree to forever discharge PIVOTAL DANCE PERFORMANCE ACADEMY from all claims that you may have or may have had arising from, or in connection with, either you or your child/children and PIVOTAL DANCE PERFORMANCE ACADEMY.
