By signing the below you give consent for your child/children to participate in Linda's learn to Swim swim program.
Teachers and instructors may take whatever disciplinary action they deem necessary to ensure the safety, wellbeing and successful conduct of the students as a group or individually in the learn to swim program.
You also authorise the teachers and instructors to obtain medical assistance which they deem necessary should an accident occur, and agree to pay all medical expenses incurred on behalf of the above student.
You have read the terms and conditions and are aware of the program for which your consent is requested.
You have/will disclose any medical information about your child and include other relevant information and details of limitations which they have for the activity concerned.
You further authorise qualified practitioners to administer anesthetic if such an event arises.

FEES
Payment for classes is paid per term. Information on the new term including dates will be sent out via email in a newsletter at least 4 weeks prior to the term commencement date. An invoice for the term will also be sent with a due date.
Term fees need to be paid in full prior to the first lesson of the term.
No credits or refunds will be given for missed classes for any reason.

MAKE UP LESSONS
Make up lessons are subject to availability and are not guaranteed.
You must notify us of a student's absence at least 2 hours prior to the lesson commencing. You can do so by marking a student absent on the Thinksmart Customer Portal App OR by contacting us via email, facebook messenger or phone.
Swimmers are not guaranteed a make-up lesson with their normal class instructor.
Fees must be paid in full before booking in a make-up lesson.
Make-up lessons cannot be changed or rescheduled once booked in.
Make-up lessons need to be done during that term and cannot be transferred to the following term.
All make-up lessons that have not been used during the term will be forfeited.
