Formula Performing Arts – 2026 Studio Agreement and Code of Conduct

This Agreement is designed to be fair, transparent, and easy to understand, while also protecting the wellbeing of our staff, students, and community. It is legally binding and governed by the laws of New South Wales (NSW), Australia, including the Australian Consumer Law (ACL), the Fair Trading Act 1987 (NSW), and other relevant legislation. We encourage open communication and are always here to answer any questions you may have.
This Agreement outlines the expectations and responsibilities involved in joining our studio community at Formula Performing Arts ("FPA"). By signing this document, you confirm that you understand and agree to the policies that support a respectful, safe, and positive environment for all students and families.
1. Yearly Registration Fee
A non-refundable annual registration fee is payable upon enrolment. This covers administrative costs including music licensing (via APRA/AMCOS), public liability insurance, and facility usage. Payment is required to secure class placement. 2026 Registration is $35.00 per student.
2. Invoicing and Payment Terms
* All payments are processed through Worldpay, which applies its own surcharge fees depending on your selected payment method (e.g. bank transfer, direct debit, or credit card).
* Please check these surcharge rates at the time of registration to clearly understand what additional fees may be added to your total invoice. You can find this on the worldpay website.
* Payment is required by the invoice due date. Late payments may incur administrative charges and may result in immediate suspension of enrolment without notice.
* Invoices are issued prior to the commencement of each term.
3. Direct Debit Authorisation
* By enrolling at FPA, you authorise automatic deduction of all fees (including tuition, costumes, and other applicable charges) via direct debit through Worldpay.
* You will receive automated reminders before scheduled debits.
* Costume fees will be distributed evenly across Terms 1, 2.
* Please ensure you are aware of Worldpay’s surcharge structure, as these fees are **not determined by FPA** and will be added to your payment total depending on your selected method.
4. Missed Classes and Make-Up Policy
Students are permitted **one (1) make-up class per term**, subject to availability.
* Bookings must be made via email.
* No refunds or credits apply for missed classes.
5. Physical Contact Consent
Dance instruction involves **appropriate and professional physical contact** for correction and support.
By enrolling, you consent to such contact as required for safe and effective teaching.
6. Media Consent
Unless a written objection is received prior to the start of term, you consent to FPA capturing and using photos/videos of students for marketing purposes across print, social media, and online platforms.
7. Term Charges and Refunds
* Fees are charged per term.
* Prorated fees apply for mid-term enrolments.
* Partial attendance or early withdrawal does not entitle you to a refund.
* Full term fees remain payable regardless of attendance level.
8. Withdrawal Policy
* All withdrawals must be submitted in writing via email.
* If a student withdraws mid-term, all fees for the term, including outstanding costume charges, remain payable in full.
* FPA reserves the right to process these via direct debit.
9. Uniform and Grooming Requirements
Students are required to wear designated FPA uniforms and appropriate footwear.
* Uniforms are mandatory for class and concert participation.
* Acrobatics students must wear form-fitting attire for safety.
* Failure to comply may result in removal from classes or performances.
10. Concert Participation
* Students must confirm their concert participation by the end of Term 1.
* A minimum of one FPA-branded uniform item is required to participate.
* Additional concert fees apply.
11. Active/Creative Kids Vouchers
We accept NSW Government Active and Creative Kids Vouchers for tuition only.
* Vouchers must be submitted before invoice due dates.
* Late submissions cannot be retrospectively applied.
12. Rehearsals/Concerts/Photo Days
* Additional fees apply for rehearsals, Concert days and costume requirements.
* FPA students/members must adhere to elevated standards of attendance, behaviour, and performance.
Participation is encouraged but not compulsory unless enrolled in the Company program.
13. Costume Fees
*Costume fees range from $80–$160 per class and are charged across Terms 1–2.
* Costumes remain the property of the student once paid in full.
* Costume fees are non-refundable after payment.
14. Family Discounts
Discounts are applied based on the number of students enrolled per family.
* These will be automatically calculated and applied to your invoice.
15. Observation Policy
To maintain a calm, focused learning space and child protection:
* Parents/guardians are not permitted in the studio during class unless enrolled in a parent assisted.
* Staff will collect students and return them to the waiting area as needed.
* Exceptions apply for scheduled viewing days or with special permission.
16. Emergency Medical Consent
You authorise FPA staff to act in the event of an emergency, including calling an ambulance or administering basic first aid.
* All medical costs (including ambulance) will be your responsibility.
* FPA holds public liability insurance but is not liable for personal injury where negligence is involved.
17. Traffic Safety and Liability
* Drop-offs and pick-ups must occur in designated parking areas.
* Students must be collected directly from the studio entrances.
* FPA is not liable for any incidents or infringements occurring out of the studio doors.
18. Code of Conduct – Students and Families
FPA is a welcoming, inclusive space built on kindness, respect, and collaboration. To help us maintain this environment, we ask all students, parents, guardians, and visitors to:
* Treat staff, students, and other families with courtesy and cooperation
* Use respectful and considerate language
* Avoid disruptive behaviour, including bullying or gossip
* Follow staff guidance and studio expectations
* Support a positive studio culture in person and online
The following behaviours are not permitted:
* Yelling, swearing, or aggressive language toward staff or others
* Harassment or intimidation (verbal, physical, or online)
* Defamatory or damaging comments about FPA, its staff, or community displayed online or in person
* Disruptive actions within studio or waiting areas
* Damage to property or facilities
If these standards are not upheld:
FPA reserves the right to respond as needed to protect the wellbeing of our staff and families.
This may include:
* A respectful conversation or written reminder
* Temporary suspension from classes or events
* Ending enrolment without refund if serious breaches occur
* Prohibiting individuals from attending future FPA activities
* Legal action, where necessary
These measures are in place not to punish, but to protect our FPA community and ensure that all students feel safe, supported, and inspired to thrive.
20. Agreement Acknowledgment
By signing this Agreement, I acknowledge and confirm that:
* I have read and understood the full document
* I agree to follow FPA’s policies and expectations
* I understand this Agreement is a legal document under NSW law
* I authorise direct debit payments as outlined
* I understand that FPA may take appropriate action if there is a serious breach of conduct
