Registration and Enrolment
It is a requirement of En Pointe Dance Academy for each family to register and pay the registration fee the first time they enroll each calendar year before participating in any of our classes. I understand that the registration fee is a non-refundable fee.
Parent Release Form for Photography and Videography
I give permission for En Pointe Dance Academy to use video footage and / or photographs of my child/ward. This usage may include (but is not exclusive to) displaying publicly, distributing, or publishing, photographs, and/or video of my child for use in materials that include, but may not be limited to:- printed materials (eg - brochures and newsletters) online and offline advertising and promotion - videos and digital images such for use on Social Media.
I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. I agree that these images may be used by En Pointe Dance Academy for a variety of purposes and that these images may be used without further notification. I do understand that any identifying information including surname and location will not be used in conjunction with any video or digital images.
Health Information and Medical Release/Waiver Form
I acknowledge that participation in dance is potentially dangerous and the is an inherent risk of injury involved. n allowing my child to participate in En Pointe Dance Academy activities, I hereby assume all the risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release En Pointe Dance Academy and it’s employees of any and all liability which may arise as a result of my child’s participation in activities at EN Pointe Dance Academy
Permission is also granted to En Pointe Dance Academy and its affiliates including Principal, Teachers and Parents to provide any needed first aid treatment prior to the child’s admission to a medical facility. In the event of an emergency, if the parent is unreachable, I understand that a teacher reserves the right to contact the ambulance and all fees associated with this will be at the parents expense.
We offer the following options for fee payment:
Direct Debit – This is our most popular and simple payment method, in which you may choose to pay your fees in monthly instalments or upfront for the term through our Direct Debit providers Integrapay. Choosing this option secures your spot for the entire duration of 2019. There is a small fee ($3.30 admin fee charged by Integrapay and is a one off fee) and paperwork must be filled out online prior to the commencement of classes. Clients who have previously paid by direct debit will still need to complete their adjustment paperwork at the beginning of this year. There are no fees charged for payment of your term fees if made this way.
Integrapay passes the following fees onto parents:
*American Express attracts a 3.5% fee.
*$4.40 failed transaction Fee
All students are invoiced based on the length of the school term. If the term is less or more than 10 weeks your invoice will reflect this.
You will not be charged for the Saturday or Monday of any long weekend. We do not run classes on this day.
If you decide to withdraw from classes mid-term for any reason, you will still be charged for the entire invoiced period.
Refunds will not be issued for missed classes due to cancellation where possible, we will endeavour to arrange make-up lessons for classes cancelled by En Pointe Dance Academy or will offer an alternative class.
I agree to these terms
Upfront Payment – you will receive your term invoice prior to the commencement of term and have until end of the previous term to pay these fees in full. Payment options include credit card (fees apply) or Internet Bank Transfer. Choosing this option secures your spot for the term only, your spot is only secure once FULL PAYMENT has been made as outlined below.
If you are joining in Term 1 your full payment will be required at time of enrolment, this includes your enrolment fee. Enrolment fees are only charged once per calendar year.
If you are joining our studio after term 1 you will be required to pay the total of your fees at the time of enrolment.
Overdue fees will accrue a late fee of 10% per week that it is past due and if fees remain unpaid in week 4 of classes students will not be permitted to participate in their lessons.
If you wish to withdraw from classes at En Pointe Dance Academy, it is a requirement that you notify the studio in writing. If you decide to withdraw from classes mid-term for any reason, you will still be charged for the entire invoiced period, fees are strictly non refundable.
I agree to these terms should I decide to pay upfront and n ot join Integrapay
Concert Costume fees.
We endeavour to keep concert costumes prices to an absolute minimum. You will be required to pay a deposit of $40 per child in term 2. This secures your place in the concert and secures a costume in each style you participate for the end of year. This deposit is non refundable.
A separate concert costume invoice will be issued and due in term 3. Any unpaid/overdue fees will incur a 10% late fee and you will not be allowed to take the costume home until all outstanding fees have been paid for.
If a student joins mid term the fees are charged on a Pro Rata Basis.
I understand that class teachers are not in a position to discuss fees and all enquries need to be directed to Miss Natalie via email – Natalie@enpointedance.com.au
We are happy for you to trial any class in your age group. This attracts a $15 fee. Once enrolled this fee is credited to your term fees. I understand that this fee must be paid to secure my trial lesson.
Photography and Digital Recoding during lessons
Due to licensing and child protection laws, En Pointe Dance Academy does not allow personal recording in any format during lessons. Digital recording of movement by any device (such as but not limited to mobile phones and digital video recorders) is strictly prohibited. Personal photograph of your own student is permitted. Posting photographs on any social media site containing images of children other than that of your own is strictly prohibited.
