Welcome to Formula Performing Arts! We're delighted to have you join us in this vibrant community of performers. Please take a moment to review and understand the terms and conditions outlined below. Your participation in our classes and your signature on this contract signifies your acceptance of these terms.

1. Yearly Registration Fee: The annual registration fee, covering music licensing, insurances, and studio materials, is non-refundable and is due before the start of classes in your first term.

2. Invoices and Payment: Invoices will be emailed before each term. You have until the due date to make payment. Late payments will incur a fee. Failure to pay may result in a hold on your spot until full payment is received.

3. Physical Contact: Dance involves physical activity, and our instructors may use gentle physical contact to assist in teaching. By enrolling, you grant permission for such interactions.

4. Photographs & Videography: We may use photographs and videos of students for promotional materials. In signing this form you consent for use of such materials.

5. Account Settlement: We appreciate timely payments. Reminders will be sent a few days prior to your due date of each term. Late fees will be applied in week 1.
Unsettled accounts may be submitted to a collection agency after week 2. Payment is available by Bank Transfer, EFT or Service NSW Vouchers. We are a cashless premises.

6, Make-Up Lessons: There are no credits for missed classes. One make-up lesson is available the week following a missed class, arranged via email with the Studio Director.

7. Class Charges: Classes are charged per number of weeks in each term.

8. Incomplete Term: If a student does not complete a term, payment is still required. Spots are held whether a student attends class or not.

9. Mid-Term Enrollments: If you join partway through a term, you're responsible for the remaining weeks of that term.

10. Grooming and Attire: All students must be appropriately groomed and wear suitable dance attire. Our studio offers a specific uniform available for purchase upon enrolment.

11. Active/Creative Kids Voucher: Vouchers may only be used for Dance Class Fees.

12. Additional Activities: Events like Concerts, Rehearsals, and Photo Days may have associated fees to cover production costs. This will be added to term fees in the appropriate term.

13. Concert Participation: It is assumed every student enrolled is participating in our end of year concert. If your student(s) will not be participating - we need to know by Term 2.

14. Term Duration: Terms may vary in week numbers, typically ranging from 10-11 weeks.

15. Elite classes are invitation only. There will be additional costs for competitions, rehearsals and performances throughout your elite journey. This will be included in the suitable term invoice.

Costumes and Uniforms
We believe in quality costumes at affordable prices.
The cost per costume ranges for REC classes, $20 - $80.
'ELITE' costumes (for competition team members) can range from $65-$150 in or around. Each performer will have a costume per routine to keep. In special cases, performers may be asked to provide their own costume.

Family Discounts and Early Bird Specials
We offer a flate rate pricing for classes and events.
Families can enjoy a discount when reaching specific class targets. Additionally, an early bird discount of 5-10% is available if paid within the specified period for registration only.

Refunds and Safety
We do not provide refunds except in extreme emergencies where a class is cancelled. We offer class credits in such cases. It's crucial to keep our admin informed of any enrollment changes.

For the safety of all involved, no photography or videography is allowed in class without permission. This ensures compliance with child protection laws and copyright regulations.

Observation
Please note that, for the optimal learning environment of our students, parents are not to observe classes. We appreciate your understanding and cooperation in respecting this policy.

COVID-19 Safety
We've implemented a strict Covid-19 safety plan to ensure a safe environment for all. Please follow the protocol for attendance and operations. In the event of a lockdown, we will continue classes online, ensuring you get full value for your fees.

Online Signature Agreement
By issuing an online signature, you confirm that you have read, understood, and agree to the entirety of this contract.
