By signing this form you are entering into a contract with Commotion Performing Arts Pty Ltd to enrol your child into classes for 2020, and pay class fees in the agreed amount on your signed payment agreement (to be filled out in person at the studio prior first lesson – full pricing schedule available at www.commotion.com.au/pricing) for all of 2020, unless the enrolment is been cancelled in writing with an official cancellation form (available from reception). If signed into a direct debit payment arrangement with Integrapay, cancellation or reduction of enrolment package can be made any time throughout the year, with minimum 2 weeks notice. Commotion will raise an invoice for the full year’s tuition, and this can be paid in one lump sum or on a payment arrangement with Intergrepay.
Membership and enrolment will continue, as will full charges, regardless of whether classes are attended or not, because a spot will be held in the classes unless we are notified in person and an official cancellation form is filled out and signed. There is no cancellation fee.
CHANGES TO ENROLMENT
If you wish to make a change to your child’s class enrolment or cancel their enrolment for the following term, you will need to fill out a CHANGE TO ENROLMENT form, which is available at reception.
Strictly NO REFUNDS under any circumstances - Except in the case of our 2-week money back guarantee, which is only available to new students. Dance fees and/or costume fees that have been paid are NOT refundable, exchangeable or transferable between family members under any circumstance.
PROPERTY DAMAGE and PERSONAL LIABILITY
Commotion Performing Arts Pty Ltd does not take responsibility for personal property left at the studio or at Commotion events and performances. Students participate in classes at their own risk. Students are under the supervision of Commotion teachers ONLY during the individual student’s timetable classes. Although all due care is taken, we cannot be responsible for students out of class, or during break in between class.
By enrolling at Commotion you accept, grant permission and waive rights usage that any photographs, audio or video recording taken of your child in classes or performances, remains the property of Commotion Performing Arts Pty Ltd and may be published without further consent for any and all advertising and promotional use, by Commotion Performing Arts Pty Ltd and our affiliated partner company Stoked!Creative Pty Ltd
Commotion reserves the right to refuse or suspend enrolment at the discretion of the Director. Appropriate and respectful behaviour is expected from students and their parents at all times. Disruptive behaviour or actions that place the reputation of Commotion at risk will not be tolerated and will result in expulsion from the studio.
