1) This is a proposed timetable, therefore classes and teachers are subject to change.

2) A $40 Enrolment Administration fee will be charged upon enrolment to secure your place in our classes. This annual fee is charged per family.

3) A $10 Insurance Fee (for accident & Injury) will be ADDED to your term 1 fees per child.


4) Please note that classes will only go ahead when minimum enrollments are reached.

5) At certain times throughout the year, Dance Desires students may have the opportunity to be photographed or filmed for publications or website or to promote Dance Desires in newspapers or other media. By finalizing this enrolment you agree to those photos/videos to be used in print, online, marketing, media and educational material.



6) Dance Desires does not issue refunds for missed classes as you are holding a position in the class, however, you are encouraged to participate in a make-up class. All classes missed may be made up in another class of any style. Classes must be made up within the term they are missed. They cannot be carried over into the next term. Consideration will be given in special circumstances.


7) If a parent chooses to cease enrolment mid term, no refund will be issued for the remaining classes, as teaching staff and studio space has been allocated based upon the number of enrolments in the class.



8) We are unable to Pro Rata fees for those families taking extended holidays during the term, however special consideration may be given to students who are required to be absent due to school commitments.

9) All students partcipating in the end of year concert will be required to pay a $60 concert levy. This will be included in your term 4 invoice. This levy covers, Digital downlaod of all concerts, all staffing at theatre rehearsal, photo day and concert. transport of all costumes and props.


10) The Parent has engaged Dance Desires to teach his/her child (“the child”) the art of dance and acrobatics. The Parent understands and acknowledges there is a risk of personal injury to the child involved in the course of instruction, participation and performance of dance and acrobatics.
To the extent permitted by law:

• Dance Desires accepts no liability or responsibility for any injury sustained by the child that arises from participation in any activity connected with Dance Desires;

• The Parent agrees to indemnify and hold harmless Dance Desires and staff employed by Dance Desires from any and all losses resulting from or as a consequence of any injury to the child in any way during participation in any activity connected with Dance Desires.

• Due to the physical demands of dance and acrobatics skills, the Parent understands and agrees that the child must be in good physical condition to participate in dance and acrobatics, and hereby certifies that the child is in an appropriate physical condition for such activities.

• In the event the child is injured, the Parent hereby authorises the staff of Dance Desires to undertake any such first aid to the child as is reasonably determined by the relevant staff member.

• Enrolment and/or attendance at Dance Desires classes constitutes acceptance of the above terms and conditions.

COVID-19 RETURN TO DANCING CONSENT FORM

Assumption of the Risk to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organisation.
COVID-19 is extremely contagious and is believed to spread mainly from person-to person contact. As a result, the Australian Government and Department of Health and Human Services (DHHS) have provided Dance Desires with clear guidelines and preventative measures which we have put into place to reduce the spread of COVID-19.
Whilst we pledge to follow every precaution necessary to provide a safe and healthy environment for our students to return to dancing, Dance Desires cannot guarantee that you or your child(ren) will not become infected with COVID-19 whilst attending our facilities or taking part in events related to our dance studio.
By accepting this agreement, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that your child(ren) and yourself may be exposed to or infected by COVID-19 by attending dance classes and that such exposure or infection may result in personal injury, illness, permanent disability, and death. You understand that the risk of becoming exposed to or infected by COVID-19 at the Dance Studio may result from the actions, omissions, or negligence of others, including, but not limited to Teaching Staff.
You also confirm that you will not allow your child(ren) to attend classes at Dance Desires if they are presenting any of the following symptoms of COVID-19 listed here, or if anyone within your family home is presenting with any of these symptoms: Fever, Shortness of Breath, Loss of Sense of Taste or Smell, Dry Cough, Runny Nose, Sore Throat

Waiver of Liability Relating to Coronavirus/COVID-19
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that myself or my child(ren) may experience or incur in connection with my child(ren)’s attendance at Dance Desires. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Dance Desires, its teachers, employees and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Dance Studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any related Dance Classes.
